Patient Office Policies

To provide timely, high-quality care to all patients, the following policies apply:

Active Patient Requirement

Patients must be seen at least twice per year to remain active with the practice. Typically, one visit is used to address healthcare needs and order any necessary testing, and a second visit is used to review results and update medications or treatment plans if needed. Patients who do not meet this requirement may be removed from the active patient panel.

No-Show Policy

Patients who accumulate three (3) no-call/no-show appointments will be released from care and dismissed from the practice.

Laboratory & Diagnostic Order Changes

Please verify your preferred laboratory or diagnostic facility before orders are submitted. Requests to change laboratories or diagnostic centers after orders have been placed will incur a $25 administrative fee due to the time required to cancel and rewrite orders.

Late Cancellation Policy

Appointments cancelled within 12 hours of the scheduled appointment time will incur a $50 late cancellation fee. This policy helps offset the loss of appointment availability for other patients.
